Community Hospice & Health Services Foundation
The mission of the Community Hospice & Health Services Foundation is to raise funds and awareness to support the programs and services offered by Community Hospice & Health Services.
As Community Hospice & Health Services continues to grow and diversify the services we offer to our patients and community, the Foundation has an opportunity to expand our philanthropic efforts to meet new evolving needs and to maintain current services. Through generous gifts, competitive grant awards, special events and new collaborations we have stood arm in arm with our donors and community partners to make a positive impact for those who are dying, seriously ill, struggling with mental health issues, grieving the passing of a loved one and more. We are thankful to our donors, Board, volunteers and Friends of Community Hospice & Health Services members who join us in our mission and know we cannot do this critical work alone without the support of our benefactors.
We are excited to share 2023 highlights of the Foundation’s outreach to the community and the special events we held to raise funds and awareness of our programs and services. What you won’t see, but what is most important, are the hundreds upon hundreds of lives that were impacted this past year because of people, perhaps you, who cared enough to make a difference. Our hearts are filled with gratitude for all who support our mission!

Foundation 2023 Highlights
Since 1979, Community Hospice & Health Services has been at the forefront of hospice and support services throughout the Central Valley. It is because of our donors we continue the critical and compassionate services and programs offered by Community Hospice & Health Services. Your gifts are visible each day through the work of our doctors, nurses, chaplains, social workers, hospice aides, grief support specialists and other program support staff. Thank you for continuing to support our vital mission.

Between her active service with the United States Air Force and as a member of the Air National Guard, Lina spend 25 years in honorable service.

Giving Tree Dedication
In the lobby of the Haig and Isabel Berberian Patient Center resides a stunning tree adorned with three colored metal leaves and branches, lovingly referred to as the “Giving Tree”. It shines with light reflecting copper, silver and gold leaves that can be engraved with names, dates or short messages for your loved one. Each year, we hold a ceremony honoring the lives represented by those on our leaves and branches. To learn more about our upcoming Giving Tree Dedication Ceremony please call the Foundation at 209.578.6370.
Golf Fore Community Hospice & Health Services
Our annual Golf Fore Community Hospice & Health Services tournament held at Del Rio Country Club was a huge success with a full field of golfers hitting the links. Thank you to Sierra Ocean Custom Wealth for providing breakfast, our numerous vendors, and sponsors. We raised nearly $104,000 to support the compassionate mission of Community Hospice & Health Services and look forward to sharing another special day with all again on Monday, September 16, 2024.

Light Up A Life Holiday Tree
The longest standing event for Friends of Community Hospice & Health Services is the Light Up A Life Holiday Tree Lighting Ceremony. This annual tree lighting event is held on the first Sunday of December at Memorial Medical Center.
Our Friends work tirelessly to make lasting memories for families by selling symbolic lights and tree ornaments in memory and in honor of loved ones. Last year, we raised over $21,000 and recognized the Stinson Family as our honorary tree lighters.
We look forward to expanding our Light Up A Life event next year to include the Oakdale and Turlock communities.
